Student Who Wish to Become part of our student leadership team need to complete this application and submit it to our Minister to Students

Students serve on the leadership team from June 1st - May 31st.
Applications for the new year are due by April 15th.
Questions can be answered by any of our Bible Study Leaders.

 

Parents (legal guardians) of Students should print out and complete both the Student Permission form and complet both the permission form and the medial form attached.  Forms must be signed in the presence of a Notary Public.
Most insurance offices and banks have a Notary Public available.
Be sure to attache a copy of your medical insurance card to the foms.
Please review the attached Group Activity Guidelines - Both Students and a Parent must sign.
We want you to know that we will do all we can to help your student have a positive experience as they
participate in our Student Ministry!  We want to see them grow in their relationships
with each other and with God and have lots of fun in a safe environment for everybody!!
